Policies and Procedures

Evaluations take place at performance evaluation review time or when a team project is completed. Chairs and team leaders complete a form for each member of the committee or team, share it with the member, and send it to the member's supervisor.

Chairs should send a link to this form to all committee members and contact all team leaders to request that they do the same for their members. Evaluations are due by March 15 of the current year.

Evaluations take place at performance evaluation review time or when a team project is completed. If you have a team that has not yet met, then the form is optional. Teams that have not completed their tasks should be included in the evaluation, as this gives them an opportunity to receive feedback and supports their review.

Chairs should send a link to the chair evaluation form to each committee member with instructions to return it to them and to their supervisor by April 1 of the current year.

Chairs should also contact all leaders of teams created by their committee. Send them a link to the form and ask them to send it to all of their team members with the instructions to return the completed form to them and their supervisors by April 1 of the current year.
